Terms of Service.

These terms govern your visits to Revelle Med Spa and your bookings through this website. They are written to be read, not to confuse.

Acceptance of terms

By booking a consultation or treatment, by purchasing a product, or by submitting any form on this website, you agree to these terms and to our Privacy Policy. If you do not agree, please do not use our services.

About our services

Revelle Med Spa is a medical aesthetics practice in Vaughan, Ontario, operated by Annoshia Siva. We provide non-surgical cosmetic treatments including injectables, intravenous wellness, and brow services.

All treatments are delivered under a written medical directive issued by an Ontario-licensed physician. Revelle Med Spa does not provide diagnosis or treatment of medical conditions outside the scope of cosmetic care.

Eligibility

To book and receive treatment you must:

  • Be at least eighteen (18) years of age
  • Be able to provide informed consent for the proposed treatment
  • Disclose all relevant medical history, current medications, allergies, and prior cosmetic treatments
  • Have no medical contraindication to the proposed treatment, as assessed at consultation

We reserve the right to decline or postpone any treatment where, in our clinical judgment, the treatment would not be safe or in your best interest. This may include during pregnancy or breastfeeding, where you are taking specific medications, where you have an active infection or skin condition in the treatment area, or where realistic outcomes cannot be achieved.

Consultation and informed consent

Every new treatment is preceded by a private consultation. At that consultation we will:

  • Review your medical history and goals
  • Examine the treatment area
  • Explain the proposed approach, the products involved, the expected outcome, and the realistic timeline
  • Explain the foreseeable risks, side effects, and downtime
  • Discuss reasonable alternatives, including the choice not to treat
  • Provide a transparent quote in writing before any treatment begins

You will be asked to sign a written consent form specific to the treatment, separate from these terms. That consent is a clinical and legal record. You may withdraw consent at any time before treatment begins.

Pricing and payment

Prices shown on this website are starting prices in Canadian dollars, exclusive of applicable taxes (HST). Final pricing depends on the units, syringes, vials, or sessions agreed at consultation and is communicated in writing before treatment.

Payment is due at the time of service unless otherwise agreed in writing. We accept [list accepted methods: Visa, MC, Amex, debit, etc.] via our processor, [processor name]. We do not store full card numbers on our systems.

Deposits. Treatments valued at [insert threshold] or more require a non-refundable deposit of [insert amount or %] at the time of booking. The deposit is applied to the final invoice.

Bookings, rescheduling, cancellations

We hold appointments for our clients individually and our suite sees one client at a time. Out of respect for everyone’s time:

  • Rescheduling and cancellations are welcome with at least twenty-four (24) hours notice. Deposits remain valid for any rebooked appointment.
  • Cancellations with less than 24 hours notice forfeit the deposit. If no deposit was taken, a late-cancellation fee of [insert fee] may apply.
  • No-shows forfeit the deposit and may be charged the full appointment value at the practitioner’s discretion.
  • Late arrivals. If you arrive more than fifteen (15) minutes late, we may need to shorten or reschedule your appointment so that the next client is not affected. The full session fee still applies.

Treatment outcomes and disclaimers

Medical aesthetic treatments are highly individual. We use established products, evidence-based protocols, and clinical judgment to give you the best chance of an excellent outcome. We cannot, however, guarantee a specific result.

Factors outside our control, including your unique anatomy, skin response, healing capacity, lifestyle, sun exposure, and adherence to aftercare, influence the outcome of every treatment. Results may take time to appear and may not match those of any photograph or case study you have seen.

All cosmetic treatments carry some risk of side effects including bruising, swelling, asymmetry, infection, allergic reaction, and dissatisfaction with the cosmetic result. The specific risks of your treatment will be reviewed with you and documented in writing before treatment begins.

Treatment fees are not refundable. If you are unsatisfied with a result, please contact us within fourteen (14) days. We will assess the area together and, where clinically appropriate, may offer a correction at no additional charge or at a reduced rate. Touch-ups offered as a courtesy do not constitute an admission of error or liability.

Photography and likeness

We may take clinical photographs for the safe delivery of your treatment. These photographs are part of your clinical record and are governed by our Privacy Policy.

We will never use your image for marketing, on our website, or on social media without your separate, specific, written consent. Consent is voluntary, can be limited, and can be withdrawn at any time.

Intellectual property

The Revelle Med Spa name, logo, website design, written content, imagery, and the “Revelle Med Spa” product line are owned by us or used under licence. You may not copy, reproduce, or use these for any commercial purpose without our prior written consent. Personal, non-commercial linking to the site is welcome.

Limitation of liability

To the maximum extent permitted by law, Revelle Med Spa, its owners, employees, and affiliates are not liable for indirect, incidental, special, or consequential damages arising from the use of this website or any treatment, except where such damages result from our gross negligence or wilful misconduct.

Nothing in these terms limits your rights under Ontario’s Consumer Protection Act, 2002, the federal Competition Act, or any other applicable Canadian consumer-protection legislation. To the extent any provision of these terms conflicts with those statutes, the statutes prevail.

Governing law

These terms are governed by the laws of the Province of Ontario and the federal laws of Canada applicable in Ontario. Any dispute will be brought in the courts of Ontario sitting in [York Region / Toronto judicial district].

Changes to these terms

We may update these terms from time to time. The “Last updated” date at the top reflects the most recent revision. Material changes will be posted prominently on this site for at least thirty (30) days before they take effect.
